The Method
Four states, and only one of them is reported
| State | What It Means | What Happens Next |
|---|---|---|
| Loaded as Intended | The plan record matches what the sponsor submitted | Nothing. This is the only state anyone reports and it is the least interesting. |
| Loaded Differently | The transaction was accepted and a rule interpreted it another way | The most dangerous state. Nobody knows, and it surfaces in a claim or an invoice. |
| Rejected | The transaction failed and was returned | Visible, but only resolved if somebody at the sponsor understands the rejection. |
| Missing | Sent and never arrived, or arrived and was never processed | Invisible on both sides. The member believes they are covered and the plan does not know them. |

Activated matters separately from loaded. A record can exist while coverage is not yet effective anywhere a provider or pharmacy would check.
